PHALLOPLASTY
(PENIS ENHANCEMENT PROCEDURES)
The Phalloplasty procedure or Penile extension is a MINIMALLY INVASIVE SURGERY requiring a short incision (1-1 ¼ “) on the pubic area on the base of the penis shaft and involves the separation of the ligament that suspends and retains the penis from the pubic bone and the partial externalisation of the ½ of the penis that lies within the male body. The increase in length varies depending on a patient's anatomy; results generally range from 1-1/2" to 2." The limiting factor is the patient's own anatomy. There is no way to predict what that length gain will be prior to surgery. But Post-op stretching is essential to success. The maximum length is obtained when the surgery is combined with a penile stretching device.
This procedure is safe and simple does not interfere with function or sensation and the healing process is short. This procedure is done on an outpatient basis. The nerves that supply sensation to the penis and the nerves that are responsible for coordinating the ejaculatory mechanism are far removed from the operative site and are therefore unaffected by the procedure.
If you’re considering penis enhancement, this information will help provide a basic understanding of what a Phalloplasty can and cannot achieve. Because each patient's anatomy is different no amount of written information can replace a personal consultation with Dr. Gomez-Mendoza and his team.

Candidates for Penis Enhancement
The best candidates for penis enhancement are men who are looking for improvement in the way they look. If you are physically healthy and realistic in your expectations, you may be a good candidate. There is a high patient satisfaction rate from penile enlargement surgery with our techniques for penile lengthening and pericavernosal and glands penile widening for girth enhancement primarily in the flaccid state with the Cymetra dermal matrix graft (Cymetra®). Almost all men who have been informed of reasonable expectations are happy and consider the surgical procedures successful.
Are the results of penile lengthening permanent?
The results of Penile Lengthening are permanent, and when combined with a penile stretching device added length will occur over time.
Are the results of Penile Widening permanent?
The results of Penile Widening by the liposuction and lipoinjection technique are frequently not permanent. Usually only thirty percent of the fat injected into the penis survives after one year. In addition, asymmetry, that is to say, unevenness and nodularity may occur. Frequently, repeat injections (touch ups) to fill in the defects are needed.
We do not perform the liposuction technique, and we no longer perform the dermal fat graft technique.
If you have the pericavernosal penile widening for girth enhancement in the flaccid state with the Cymetra technique the results are much more predictable and permanent. And you may expect an approximate 30% width gain (circumference) in the flaccid state and an approximate 10% gain in the girth in the erect state.
We have been using the Cymetra technique for 4 years and we can tell you from our experience that the results are predictable and appear to be permanent.
What can I expect from glans enhancement surgery?
Although greater than 30% increase in the flare of the glans is possible, a reasonable expectation is between 10% and 20% increase in the flare of the proximal 1/3 of your glans (head of penis).
What type of anesthesia is used?
Dr. Gomez-Mendoza performs most phalloplasties; under sedation (gentle, short-acting medication administered by a board certified anesthesiologist to insure comfort and security for our patients) and local anesthesia. Patients are under monitored anesthesia care since they arrive to the operating room till they leave the recovery area. You will receive the safest care possible. The expert care you deserve. And our patients recover more quickly and with less discomfort than when general anesthesia is used.
Where will your Phalloplasty be performed?
Dr. Gomez-Mendoza performs most Phalloplasties at the modern Puerta de Hierro Medical Center. After your surgery…
You will wear a gauze dressing and your penis will be bruised, swollen, but the pain will be mild. Any discomfort you may feel can be relieved with your medication.The stitches in your pubic wound will be absorbed and they are located intradermically so you will not see them.
How long after surgery can I have sex?
It is advisable to wait six weeks before engaging in penetrative sex. However, you can masturbate in approximately 1 week after surgery, as long as you use gentle masturbation of the glans (head of the penis) only.
What are the risks of surgical penile lengthening or widening with your techniques?
The risks of Penile Lengthening surgery are very low with our technique. Very few patients experience complications. However, I will discuss the following possible risks with you:
• a reduction of the erect upward angle of the penis
• a keloid scar at the site of the incision
• fibrosis of a superficial vein
• skin incision separation
• bruising
• bleeding
• infection
• temporary swelling of the penis
• a collection of serum under the skin
The risks of Penile Widening surgery are very low with our techniques. Very few patients experience complications. However, I will discuss the following possible risks with you:
• absorption, contracture or failure of the graft to take
• fibrosis of a superficial vein
• infection
• bleeding
• bruising
• temporary swelling of the penis
• a collection of serum under the skin
There are not risk free surgical procedures.
